The following services and care are provided in the daily rate, no additional charges are assessed to the individual for these services
3 meals and snacks daily including some special diet considerations
Private and Semi Private accommodations
All utility costs except private telephone line
Cable TV
Daily/weekly laundry and cleaning services
24-hour care and supervision including night time assistance as requested
Coordination of all home health care services including; nursing, occupational therapy, physical therapy, speech and hospice care
Personal Care Services: Supervision to Moderate Assist at the time of admission
Moderate assist is defined as a client being able to do 50% of a task with staff assisting with the other 50%. Additional charges may be incurred if a client exceeds this level of care. Clients will be referred to a higher level of care facility if a client’s level of care can not be met safely at the program.
Assistance with:
Showering and bathing
Walking and one person assisted transfers
Oral hygiene
Dietary intake
Dressing and daily grooming
Bowel and bladder programs – clients must be cooperative and level of assistance must be manageable within the context of the program
Monitoring of oxygen orders, including reordering and minor equipment maintenance
Medication management, including daily assistance, reordering, monitoring for compliance with daily regime
Emergency response and first aid intervention
Compliance with physician orders on special request, i.e. daily weight, blood pressure etc.
Referral to physicians or specialists for specific problems
End of Life Care is provided by the program with the support of Hospice Agencies
Temporary medical needs can be met through coordination of services with Home Health Care Agencies
